To understand how immediate implantation differs from the classic approach and what advantages it has, we are joined by Pavle Khujadze, prosthodontist and implantologist at the Nino Beridze Orthodontic Center.

What is immediate implantation?

Single-stage or immediate implantation is a tooth restoration procedure in which an implant is placed in the socket right after the tooth is extracted, all in a single visit. Immediate implantation is most in demand in the front teeth area, where aesthetics matter most.

The immediate method significantly shortens rehabilitation time and allows the patient to have a temporary restoration fitted.

What types of implantation are there?

There are 3 types of this implantation procedure.

First – immediate implantation in which, after the implant is placed, the gum is sutured and further procedures are carried out only after complete healing.

Second – in addition to the implant, the patient is fitted with a gum former, which spares them from having this procedure later and also helps preserve the so-called “pink aesthetics”, i.e. a natural appearance of the soft tissues at the implant site.

Third – a temporary crown is placed on the implant inserted in place of the extracted tooth. After the implant heals, the temporary crown is replaced with a permanent one. From an aesthetic point of view, this type of immediate implantation is currently considered the most effective. With it, the patient doesn’t have to go without a tooth. This express implantation method is mainly used in the front teeth area.

Immediate dental implantation and its advantages

Creating a beautiful appearance, especially in the front area, is no less important than chewing and speaking, and the gum contour is no less important than the correctly chosen color and shape of the tooth. The advantage of immediate implantation is that, whether we place a gum former or a temporary crown, we preserve the contours of the soft tissues. With two-stage implantation, we have to create these contours artificially.

What are the contraindications for immediate implantation?

Unfortunately, immediate implantation is not always the best treatment option, because it isn’t always possible to place an implant right after extraction. For example, immediate implantation is not performed when there is a focus of inflammation at the site of the extracted tooth. Other contraindications include decompensated diabetes, severe bruxism or uncontrolled teeth grinding, some chronic diseases in the acute or relapse phase, and heart attacks, strokes or hypertensive crises within the past six months. Smokers are also at risk. Although smoking is a relative contraindication, it can very well cause implant rejection.

In other cases, immediate implantation after extraction is acceptable, but immediate loading – i.e. placing a temporary crown – is not. Such situations include, for example, traumatic tooth extraction, insufficient volume of soft and hard tissue, unreliable implant fixation and soft bone structure. In these cases, at most a gum former is placed, and if the tooth is in the aesthetic zone, a prosthetic restoration is temporarily fixed to the neighboring teeth.

Risks associated with immediate implantation

To be honest, the risks with immediate implantation are higher than with two-stage implantation, though the difference in percentage terms isn’t large. Moreover, this risk is justified when we need an immediate, highly aesthetic result.

Implant rejection may be caused by insufficient fixation and attaching a temporary crown. It’s also worth noting that if the wound isn’t sutured after extraction, it may become infected by plaque, which leads to peri-implantitis.

The doctor must correctly assess the condition of the mouth and decide whether to load the implant immediately or wait until it heals.

Learn more about immediate dental implantation.
